Care Provider Vetting Request

Use this form to ask Simply Vetting to carry out safer recruitment checks on a candidate you intend to engage. You remain the employer and responsible for the final recruitment decision. Please make sure you have made the candidate a conditional offer and told them that Simply Vetting will contact them.

6Declaration by the care provider

On behalf of the organisation named above, I confirm that:

  • the role is eligible for the level of DBS check requested, including any barred list check, under the Police Act 1997 and the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975;
  • the candidate has been given a conditional offer and told that Simply Vetting will carry out checks on our behalf and will share the outcome with us;
  • we have a lawful basis to share the candidate's details with Simply Vetting and to receive the outcome of the checks;
  • we will handle any information we receive in line with UK data protection law and the DBS Code of Practice;
  • we accept Simply Vetting's terms of business and fees for the checks requested;
  • we understand that Simply Vetting provides a vetting outcome only, and that the decision to employ or engage the candidate, and our ongoing safer recruitment duties, remain our responsibility.
